Led by Dr. Elias Sayour, a pediatric oncologist and researcher at UF Health, the team created an experimental mRNA vaccine that activates the immune system in a powerful way &#8211; strong enough to destroy tumors, even without targeting any one specific type of cancer.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThis paper describes a very unexpected and exciting observation,\u201d Dr. Sayour said. \u201cEven a vaccine not specific to any particular tumor or virus\u2014so long as it is an mRNA vaccine\u2014could lead to tumor-specific effects.\u201d The study, published in <em>Nature Biomedical Engineering<\/em>, shows how combining this vaccine with immune checkpoint inhibitors can create a powerful antitumor response in mice.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">What Is a Cancer Vaccine?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Most people think of vaccines as tools to prevent infections, like those for the flu or COVID-19. But a cancer vaccine works differently. Instead of preventing illness, it teaches the immune system to recognize and attack cancer cells already growing in the body.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dr. Sayour\u2019s team used messenger RNA, or mRNA, to develop the vaccine. This type of RNA is already found in our cells and helps direct the body to make proteins. In this case, the mRNA is designed to deliver instructions to the immune system to activate and respond with force\u2014just as it would when fighting off a virus.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The vaccine does not focus on a specific cancer-related protein. Instead, it stimulates the immune system broadly. One way it does this is by increasing the expression of a protein called PD-L1 inside tumors. PD-L1 normally helps tumors hide from immune cells. But in this study, making tumors express more PD-L1 actually made them easier for the immune system to detect and attack when combined with certain immunotherapy drugs.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">How It Works: A One-Two Punch<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The mRNA vaccine works by using lipid nanoparticles to carry the fragile RNA into the body. Once inside, the mRNA prompts the production of immune-stimulating proteins. These proteins help &#8220;wake up&#8221; T cells\u2014immune cells that can recognize and kill cancer cells. When combined with drugs known as immune checkpoint inhibitors, such as PD-1 blockers, the results were even stronger.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Checkpoint inhibitors are a type of immunotherapy that helps the immune system recognize cancer cells as dangerous. These drugs have been used to treat cancers like melanoma and lung cancer, but they don\u2019t always work. In this study, when the checkpoint drugs were paired with the new mRNA vaccine, the immune system responded much more aggressively, attacking tumors that had previously resisted treatment.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThis finding is a proof of concept that these vaccines potentially could be commercialized as universal cancer vaccines,\u201d Dr. Sayour said. \u201cThey can sensitize the immune system against a patient\u2019s individual tumor.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The research team tested the vaccine on mice with several types of cancer, including melanoma, brain tumors, bone cancer, and skin cancer. In some of these cases, the tumors were completely eliminated &#8211; even without any help from additional drugs or treatments.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The most dramatic results came from models of melanoma and glioblastoma. Glioblastoma is an aggressive brain cancer that is almost always fatal. In an earlier trial led by Dr. Sayour\u2019s lab, a personalized mRNA vaccine created from a patient\u2019s own tumor cells helped the immune system mount a strong response against glioblastoma. The new study took that idea further by removing the need to personalize the vaccine. Instead, they used a generalized mRNA formula that worked broadly across tumor types.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dr. Duane Mitchell, a co-author of the paper and director of UF\u2019s Clinical and Translational Science Institute, explained the shift: \u201cUntil now, there have been two main ideas in cancer-vaccine development. One is to find a specific target expressed in many people with cancer. The other is to tailor a vaccine specific to targets expressed within a patient&#8217;s own cancer.\u201d This new approach, he said, \u201csuggests a third emerging paradigm.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cWhat we found is by using a vaccine designed not to target cancer specifically but rather to stimulate a strong immunologic response, we could elicit a very strong anticancer reaction,\u201d Dr. Mitchell said. \u201cThis has significant potential to be broadly used across cancer patients\u2014even possibly leading us to an off-the-shelf cancer vaccine.\u201d<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Is It a Cure?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The idea of a universal cancer vaccine might sound like science fiction, but these results bring it a step closer to reality. In the mice studied, the vaccine helped eliminate existing tumors, suggesting it could serve as a treatment\u2014not just prevention. While this doesn&#8217;t mean we have a cure yet, it raises hopes for a future where cancer might be treated with a simple injection that prompts the body to do the work itself.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Scientists believe this approach could eventually replace or reduce the need for surgery, chemotherapy, and radiation, which are often painful and have serious side effects. \u201cThis could lead to a new way of treating cancer without relying solely on surgery, radiation, or chemotherapy,\u201d Dr. Sayour said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This research builds on technology that became widely known during the COVID-19 pandemic. mRNA vaccines from companies like Pfizer and Moderna showed how quickly and effectively mRNA could be used to train the immune system. That success helped accelerate interest and funding for mRNA research in other areas\u2014including cancer.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dr. Sayour has spent more than eight years working with mRNA and lipid nanoparticles to create anticancer treatments. Now, his team is working to improve the current formulations and move into human clinical trials as soon as possible.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">While the vaccine is still being tested in animals, the results are encouraging enough to suggest that human trials may not be far off. If successful, this universal vaccine could be a game-changer in the fight against one of the world\u2019s deadliest diseases.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">A New Era in Cancer Treatment?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cWe\u2019re not targeting one kind of tumor,\u201d Dr. Sayour explained. \u201cWe\u2019re targeting how to give the body the tools to fight any kind of cancer cell.\u201d That idea challenges the current model of personalized medicine, which focuses on tailoring treatments to each patient\u2019s specific cancer profile. Instead, this approach could offer a simpler, more accessible solution for people all over the world.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cIt could potentially be a universal way of waking up a patient\u2019s own immune response to cancer,\u201d Dr. Mitchell said. \u201cAnd that would be profound if generalizable to human studies.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If further testing proves successful, this vaccine could offer hope not just for patients with hard-to-treat cancers but for the future of medicine itself.
